Lasting Power of Attorney (LPA) – Singapore
Plan Ahead. Protect Your Future.
At Ultimed, we believe in empowering individuals to make informed decisions about their healthcare and personal welfare — not just for today, but for the future. That’s why we support and guide our patients through the Lasting Power of Attorney (LPA) process in Singapore.
What is an LPA?
A Lasting Power of Attorney (LPA) is a legal document that allows a person (known as the donor) to appoint one or more persons (donees) to make decisions and act on their behalf should they lose mental capacity in the future.
The LPA ensures that your personal welfare, healthcare decisions, and/or property and financial matters are handled by someone you trust.

Who Should Consider an LPA?
Anyone aged 21 and above with mental capacity can apply for an LPA. It is especially important for:
• Seniors
• Individuals with chronic medical conditions
• Anyone who wants to ensure future decisions are made by trusted loved ones
When is an LPA Used?
The LPA only takes effect if the donor loses mental capacity, for example due to:
• Stroke
• Dementia
• Traumatic brain injury
• Severe illness

How Ultimed Can Help
Our doctors are all accredited LPA Certificate Issuer and can assist with:
• LPA Certification: Verifying that the donor and donee understand the purpose and implications of the LPA and are mentally fit to make decisions pertaining to LPA, and are not under duress.
• Mental Assessments (if needed)
• Guidance on Application through the Office of the Public Guardian (OPG)
Types of Decisions Covered in LPA
There are two categories you can authorize your donee(s) to manage:
1. Personal Welfare
Health, daily care, accommodation, and medical treatment
2. Property & Affairs
Managing bank accounts, paying bills, or selling property
Why Make an LPA?
• Ensures your wishes are respected if you lose mental capacity
• Reduces stress and legal hurdles for your family
• Avoids costly court applications under the Mental Capacity Act
• Gives you peace of mind
