Privacy Statement 

ULTIMED is a licensed healthcare provider under the Healthcare Services Act.   

 

ULTIMED, its authorised healthcare professionals, and/or authorised vendors, may generally collect, use and disclose personal data (including health information), that you knowingly and voluntarily provide directly to us or via a third party who has been authorised by you to disclose your personal data to us. 

 

Collection, use and disclosure of your personal data may also be permitted or required by the Health Information Bill, Personal Data Protection Act 2012 or other relevant laws, rules or regulations.  

 

Health information includes:  

 

  • Administrative data which includes the patient's personal information such as name, address, contact details, as well as other demographic data. Such date is only considered as health information if used in relation to the provision, use or consumption of healthcare. 

 

  • Clinical data namely, all information about or in relation to (i) the physical and mental health of the patient, or (ii) the diagnosis, care, and treatment of the patient.  

 

  • Sensitive health information which includes sexually transmitted infections, mental disorders, substance abuse and addictions, biological parenthood, voluntary sterilization and termination of pregnancy, organ donation, suicide and abuse.  

 

Before we collect any personal data from you, we will generally notify you of and obtain your consent for the purposes for which your personal data may be collected, used and disclosed, including but is not limited to the following:  

 

  • Provision of a healthcare service to you as an individual or any administrative matter directly related to the provision of such service, such as scheduling of appointments with ULTIMED, and transfer or referral of the individual to another care provider who may provide a healthcare service to the individual.  

  • Contacting you or communicating with you via various modes of communication such as voice call, text message or fax message, email or postal mail for the purposes of administering and managing your relationship with us. 

  • Complying with or as required by any request or direction of any governmental authority or responding to requests for information from any governmental authority, public agencies, ministries, statutory boards or other similar authorities including their appointed agents, contractors or third party service providers.  

  • Complying with or as required by any applicable law, court order, governmental or regulatory requirements including but not limited to the compliance with the disclosure requirement.  

  • Arranging, administering, or executing audits and governance reviews.  

  • Executing business or service improvement, quality assurance reviews, surveys or research.  

  • (wherever CCTV or other surveillance equipment are deployed on site), for detection and prevention of crime and for security purposes and investigations; 

  • Engaging with third party vendors in respect of procuring goods or services incidental or necessary to the provision of services to you; 

  • Any other purposes which ULTIMED may inform you in writing from time to time, but for which ULTIMED will seek your separate consent. 

 
Where you are deemed by law to have consented to various purposes, these purposes will be acted on until such time as such deemed consent is withdrawn in accordance with the arrangements we have in place to assist your doing so.  

 

Your failure to supply certain personal data to us (or if we are not permitted by you to process your personal data), may/would result in us being unable to process, administer and/or manage your relationship with us. Depending on the extent by which you do not permit us to process your personal data or the extent of personal data which you do not provide to us, it may mean that we will not be able to continue the relationship with you, as we will be unable to administer the relationship.
